Reliability is incredibly important.

If something fails you just once, accidentally, it will have deeper implications than just the immediate cost.

It might mean that you stop trusting software that failed you once. It might mean that you won’t count on an employee that made a reckless mistake. It might mean that you find a different job if management doesn’t have your back this time.

Even though it might seem like a fluke, like a silly mistake or an accident, make sure everyone affected by it understands what happened and why. Then take all necessary steps to prevent it from happening again.
